Our student forms are transitioning to a new and easier format called Etrieve. As
we introduce new eforms, we will update the links below which will send you directly
to Etrieve to complete your request. Once you fill out the form and submit it, the
form is automatically sent where it needs to go! See tutorial below to access your
history of submitted forms, drafts, and all Etrieve forms.

You must download and save your document before filling it out. If you don't, you
will send back a blank document. You must send your completed form using your Columbia
College email address or it will not be processed.
To create an e-signature: click on the signature field, choose draw, type, or upload,
and then click apply to save.
- Request credit from previous accredited colleges that you attended to be added to
your Columbia record. A&R must have already received your official transcript to
process this request. If you need help completing this request, please make an appointment
with a counselor 209-588-5109.
Dual Enrollment registration forms will only be accepted if signed by a high school
official. Unsigned registration forms will only be accepted if emailed from a high
school official's email address giving us permission to process them. Please visit

AB 13 VACA Affidavit- Veterans and dependents use this form to update residency.
- Apply to have your "non-California resident' status reviewed. If you have attended
three (3) years of K-12 in California AND graduated from a California high school,
or, if you have attended three (3) years of K-12 in California AND received a GED,
from a California high school.
Application for Course Audit- Students must have taken and passed the course, and must have met the maximum number
of repeats in order to petiiton to audit. Students may not start the petition process
until the first day of the course.
Course Drop Form- Use this form to request to drop a course with assistance from A&R.
- Use this form when you need to add a class with assistance from A&R. If you can't
log in to Etrieve, you may use this .
- Apply for a refund of the student activity sticker fee. The Student Activity Fee
is used to support student events and activities on campus, such as Cram Night, free
student BBQ's, movie nights and other free activities. It also pays for scholarships,
clubs and sponsorships. Please use this form if, for religious, political, financial
or moral reasons, you would like to request a waiver of the student representation
fee.
- Use this form to make a legal name change, or, correct a birthdate or social security
number. You can update your address and personal email yourself in connectColumbia
without a form.
- Request a refund when you have a credit on your student account.
Release of Student Information- You and the person you are releasing personal and confidential information to must
appear in person to Admissions and Records with photo I.D.
- Application for non-California resident status review.
- Application to appeal a residency determination submitted via Residency Status Consideration
request.
Student Parent Priority Registration -Request priority registration if you are a single parent who receives more than
half of your support for your child/children.
Students must apply for graduation. To obtain a award, you will need an approved degree plan in Starfish which can be obtained by meeting with a counselor. Schedule an appointment with your academic counselor. Or, contact the General Counseling office: 209-588-5109 or Special Programs office 209-588-5130.

- Petition to alleviate substandard grades from GPA computation. See catalog for
requirements.
- Petition to grant credit for work experience, military experience, special training,
etc. In order to receive course credit, students must prove that they have met the outcomes of the Course Outline of Record. Students should reach out to the faculty that teaching the course so that they can
share the course outcomes with you. See theCPLweb page for requirements.
- Petition to repeat a course that exceeds maximum allowable course enrollments.
- Petition for course substitution or waiver for a degree or certificate. No units
will be given. If approved, the requirement will be waived. This form should be
filed prior to taking a course for substitution. Consult with your counselor. Approved
substitutions or waivers will be displayed on your degree plan in Starfish.
General Student Petition- Petition used to request a drop with or without a withdrawal (W), excused withdrawal
(EW), military withdraw (MW), and to request a registration hold exemption. Note-If
you attended your course AFTER the census date, you cannot have a W grade removed.
- Apply for Pass/No Pass grading status for a class. See catalog for deadlines.
Petitions Appeal- Appeal the decision that has been made on my student petition.
- Petition to enroll in a course when you have not completed the required prerequisite
or corequisite course. Please allow at least two weeks to process.
- Before you use this form, contact your instructor. If the grade dispute remains
unresolved, you may file this form.
Time Conflict Petition - Used when a student tries to enroll in two classes that coincide in times (beginning
and/or end times) and require work to be made up in either course. You must also
submit a to be registered with an approved petition.
- Used to correct a grading error after submitting final grades.
Incomplete Grade Request - Allow a student an incomplete grade at the end of the semester. The instructor
must inform the student they have been given an incomplete grade. Instructors must
also contact Admissions and Records if a grade needs to be changed/updated before
the expiration date of the request.
- Request to add a student to a course after census.
- Request to drop a student from a course after census.
- Allow a dropped student to return to a course they were previously attending.
